Insurance Information

I am contracted with Blue Cross Blue Shield to accept the following insurance plans as an in-network provider: Independence, Personal Choice, Keystone Health Plan East and Independence Administrators as well as AmeriHealth and AmeriHealth Administrators. I am also considered in network with all out of state Blue Cross plans.:

If you have an insurance carrier that I am not in-network with, I can verify your out-of-network benefits for acupuncture.  If you have out-of-network acupuncture benefits, we will submit a claim to your insurance company on your behalf. 

If requested I can provide a super bill via e-mail for you to submit to your insurance company.

Payment Information

Payment is due at the time of service.  I accept credit cards, Venmo, check, cash, FSA/HAS card payments as payment.
